To facilitate the handling and review of your complaint, please complete a "complaint summary form". You should provide all relevant information and documentation when this form is completed, including a copy of any correspondence you have received from your mortgage broker's company which explains their position on your complaint.
A copy of the complaint summary form can be downloaded and printed by clicking on the pdf file below.
When your complaint is received, it will be replied to and handled as quickly as possible by FSCO's mortgage brokers' area.
